- Grand Slam Donation Level $5,000
- Home Run Donation Level $2,500
- Triple Play Donation Level $1,000
- Double Play Donation Level $500
- Single Play Donation Level $250

Grand Slam Donation Level $5,000 +
- The opportunity for you and three others during the 2013 regular season (excluding Opening Day) to view batting practice on the field, to see your name in lights on the Indians Scoreboard and to visit the Radio and TV booths (one-half inning in each booth).
- A drawing from the pool of Grand Slam Level donors to win an Indians Suite (12 tickets/3 parking passes; no food and beverage) for one regular season game during the 2013 season (excluding Opening Day).
- 5 tickets for any Indians event with baseball celebrities.
- Sluggers Cocktail Event prior to any fundraising function
- Full page advertising on the new Baseball Heritage Museum Website
- Commemorative Baseball Cap and T-Shirt
- Signage in the Museum
- Autographed Leon Day tribute poster
